Live 5-2-1-0 Script for Service Providers

Through its partnership with SCOPE and Fraser Health, Healthy Abbotsford developed a script that was originally used by Public Health Nurses to introduce the Live 5-2-1-0 message to new families (more information on this initiative can be found in the ‘Health’ Sector). This Live 5-2-1-0 script is also useful for other Community Service Providers, who can use it to introduce and discuss the Live 5-2-1-0 message with children and families in a number of settings.

Please note: The Script for Service Providers is the same resources as the Live 5-2-1-0 Public Health Nurse Script, they differ only in name.
