Information for Healthcare Providers
Supporting health behaviour change in families can be challenging. Even when working together to set goals during appointments, it can be difficult to motivate patients to continue working on these in between visits.
The Live 5-2-1-0 App guides children through the steps of building weekly health goals – kids can focus on healthy eating, increasing physical activity, reducing screen time, and more. All activities can be tracked along the way, and fun rewards (chosen and provided by families) can be earned for completing goals.
Measuring Behaviour Change
Children are guided through a series of 10 assessment questions upon downloading the app. The assessment questions ask children about their daily health behaviours and willingness to change, in five categories – vegetable & fruit consumption, screen time, physical activity, sugary drink consumption, and sleep. The app will also prompt the child to retake the assessment every 90 days, allowing changes in behaviour to be recorded in the app. Users can retake the assessment by waiting for the 90-day prompt, or by manually selecting the ‘Retake Assessment’ button on the Progress Report, Assessment tab. Healthcare providers may wish to ask their patients to retake the assessment before follow-up visits.
Dashboard for Healthcare Providers
The app features a dashboard for healthcare providers called My Progress Report. The report allows for a quick glimpse of the patient’s progress at follow-up visits. The Progress Report includes two tabs; Goals and Assessments.
Take a look at your patient’s device to see:
Goals
- The number of activities and goals completed in each category: 1. A complete history of the goals set by the patient, and 2. The reasons why goals were changed or not completed
Assessments
- Changes over time in the patient’s self-reported daily health behaviours and willingness to change these behaviours
