Building a network of partners and champions with a shared vision for change is important to ensure success of Live 5-2-1-0. By engaging a diverse group of stakeholders, change will happen at many levels, across different settings, which maximizes impact.
Evidence has shown that this approach works – similar community engagement programs such as those in Maine and Massachusetts have resulted in positive changes in children’s health. And while it’s a proven approach, we know that every community is unique – so it’s essential that local knowledge and leadership are what guide the process.

The path to sharing and supporting Live 5-2-1-0 successfully is not a straight line – the process looks a little bit different in every community. However, there are some important common steps that need to happen.

The Live 5-2-1-0 Community Road Map (embedded below) describes these necessary common steps, and provides guidance to those interested in bringing Live 5-2-1-0 to the broader community in which they reside.

The Live 5-2-1-0 Community Road Map outlines adaptable approaches for engaging and empowering local decision makers, based on best practice and the experiences shared with us from other Live 5-2-1-0 communities. We encourage communities to review the Road Map together with their local partner groups, so that they can use it to help guide collaborative planning.
