Live 5-2-1-0 Public Health Nurse Script

In partnership with Live 5-2-1-0 and Fraser Health, one of the very first initiatives implemented by Abbotsford, also one of the first Live 5-2-1-0 communities, was the Public Health Nurse Live 5-2-1-0 Script for childhood immunization visits.

During Abbotsford’s process of community needs assessment and action planning, stakeholders identified a great opportunity to consistently educate new parents about the benefits of helping their children make healthy choices. In Abbotsford, all new parents see a Public Health Nurse during their children’s regularly scheduled childhood immunizations. Because this service is centrally coordinated in Abbotsford through their Public Health unit, it was possible to roll this initiative out across all childhood immunization clinics, which expanded the reach of the message to all new parents in Abbotsford.

In order to do this, Abbotsford and their Public Health unit of Fraser Health jointly developed a Live 5-2-1-0 Script for Public Health Nurses to use, which provided them with a simple and easy way to deliver consistent, evidence-based information. Partners at Public Health held trainings for the nurses and provided copies of the script.

The Live 5-2-1-0 Script introduces the Live 5-2-1-0 message to parents, explains why each of the guidelines is important for children in an easy-to-understand way, and offers some tips on how to achieve these guidelines.
