Live 5-2-1-0 Family Physician Toolkit: Readiness Ruler
Live 5-2-1-0, in partnership with the Chilliwack Division of Family Practice, developed and piloted a Live 5-2-1-0 Family Physician Toolkit which aimed to support Family Physicians in making BMI assessment and healthy living discussions part of regular family practice visits with children and youth. The Toolkit is based on existing evidence-based best practice, and provides information and resources to support family physicians in accurately measuring and classifying BMI percentiles in pediatric patients, assessment of healthy and unhealthy lifestyle choices, respectful discussions around healthy behaviours, motivational interviewing to support effective behaviour change, and other resources available locally to support families in making healthy choices.
The Live 5-2-1-0 Readiness Ruler is one of the three key components of the Live 5-2-1-0 Family Physician Toolkit, which is aimed to support Family Physicians in making healthy living discussions a part of regular clinical visits with children and youth. The Readiness Ruler serves as a decisional balance tool for healthcare providers to use with their patients during clinical encounters when discussing behaviour change and their readiness to change.
