Live 5-2-1-0 Family Physician Toolkit: Guide to Respectfully Discuss Weight with Pediatric Patients

The Healthcare Provider’s Guide to Respectfully Discuss Weight with Pediatric Patients and their Families was developed as part of the Live 5-2-1-0 Family Physician Toolkit. This resource supports healthcare providers in identifying their own implicit biases before initiating conversations about weight, and provides evidence-based tips and tools to discuss weight in a constructive and non-stigmatizing manner.

Live 5-2-1-0, in partnership with the Chilliwack Division of Family Practice, developed and piloted a Live 5-2-1-0 Family Physician Toolkit which aimed to support Family Physicians in making BMI assessment and healthy living discussions part of regular family practice visits with children and youth. The Toolkit is based on existing evidence-based best practice, and provides information and resources to support family physicians in accurately measuring and classifying BMI percentiles in pediatric patients, assessment of healthy and unhealthy lifestyle choices, respectful discussions around healthy behaviours, motivational interviewing to support effective behaviour change, and other resources available locally to support families in making healthy choices. Other key elements of the Toolkit include:

  • The Live 5-2-1-0 Assessment and Management Flow Chart (based on the ‘Pathway for the Identification, Assessment and Management of Overweight and Obese Children & Youth’ developed by Child Health BC in the Consensus Meeting on Weight Management for Children and Youth Final Report)
  • The Live 5-2-1-0 Healthy Habits Questionnaires (supporting tool for use with the Assessment and Management Flow Chart)
  • The Live 5-2-1-0 Readiness Ruler (supporting tool for use with the Assessment and Management Flow Chart)
