The Live 5-2-1-0 App is a FREE resource that supports children in building healthy habits while working towards the Live 5-2-1-0 guidelines. Families can download the app from the App Store or Google Play.

Note: As a healthcare provider, you do not need to have the app installed on your device. You can track the patient’s progress with goal-setting and behaviour change by viewing the ‘My Progress Report’ page of the app on their personal device – a visual tool to assess change at follow up visits.
